Who is this service for?
For people aged 18 and over who are registered with a North Somerset GP. District nursing care (part of the community team) will only be provided to individuals who need an ambulance to leave their home, or where a home visit is appropriate.
The community ward, physiotherapy and occupational therapy are provided to people regardless of whether they are housebound or not.
What we do
We want to support you and your carers to manage your individual health needs, help you to maintain your quality of life, and live as independently as possible.To do that we have many community teams; a group of healthcare professionals working in the community in partnership with your GP and other services such as social care.
These teams consist of a range of clinical staff including:
- Nurses
- Occupational therapists
- Physiotherapists
- Support workers
How do I access this service?
You need to be referred by a healthcare professional to access this service.
Cancelled/missed appointments
If you cannot make your appointment, we require a minimum of 24 hours’ notice to reschedule your appointment. Please contact us on 01275 888801.
You may be discharged if you miss any appointments without letting us know, or if you cancel appointments more than once. A discharge letter will be sent to your GP who can re-refer you if they feel this is appropriate.
