Access Your Care was established in 2011 and has since supported over 13,000 satisfied clients.  The company ethos is based on innovative, flexible and personalised care and support services which are provided across the district.  Over the last few years we have developed new services working in partnership with our clients, the local authority and health partners – together we are reshaping the care sector.

The services we provide are designed to suit each person’s individual requirements and goals, ranging from the provision of personal or practical support to meeting complex care needs.  We provide these services to people 18 years of age or over.  We are registered with the Care Quality Commission and we are a member of the United Kingdom Home Care Association. 

Access Your Care is committed to offering responsive services shaped for the local community by the local community.  

 

Our services include:  

 

Home Care – Day time

Our CareHub home care service has been created to offer a ‘one stop shop’ for all home care and domestic needs. 

Personal Care

Bathing; washing; showering; assistance with continence care; assistance with dressing and undressing; medication management; post operation care; night care

Meals    

Preparing meals; planning meals; cooking meals; assistance with eating and drinking

Day Sitting & Companionship Care    

This service is often used to give family members or the informal carer a break.  Both practical support and personal care tasks can be carried out during this time

Night Care        

We can offer ‘traditional’ waking or sleep-in services, or why not opt for our innovative cost saving overnight care visits service

End of Life Care

Specialist personal care and support at a critical and sensitive time

Hospital Discharge

Individualised care and support when you return home from hospital and need to recuperate from an operation or illness.  We will provide short term care and support until you are settled back at home and have regained your independence

Respite Care            

For people caring for the cared for.  A service that can be planned or put in place at short notice, to provide a vital lifeline to those who need a break from their caring role

 

Home Care - Overnight

As a company we specialise in overnight care services including planned care visits and night sitter services.

Schedule Planned Visits

This service provides two carers visiting the individual’s home at pre-arranged times during the night.  The carers can visit more than once in the night depending on the needs of the individual.

Sleeping Night

This service provides one or more night time carer/s sleeping at the property for a minimum of eight hours. This option will be best for anyone who doesn’t require regular support but may feel uncomfortable when left alone throughout the night.

Waking Night

This service provides one or more night time carer/s remaining awake all night. The carer/s are on duty throughout the night and for a minimum of eight hours, but this is flexible. This may be the best option for anyone who wakes several times throughout the night needing support.

Some of the person centred tasks provided by our overnight service are:         

  • Toileting support                                       
  • Welfare visits
  • End of life support
  • Medication support
  • Confidence & independence support  
  • Support for carers and/or family members                                           
  • ‘Specialised techniques’ for example: PEG feed, Stoma care, catheter care
  • Re-abling individuals to regain their independence following a hospital discharge or a downturn in their personal health and/or situation

 

Response24 

A 24 hour emergency care response service which can be linked with any monitored alarm system and/or Telecare equipment, offering a first responder service throughout North Somerset, the service can be accessed for both an all year round and/or weekly cover dependent on your needs.  Peace of mind for individuals and family members 24 hours a day, 365 days a year.  During a response call, we can offer welfare checks, personal care and assistance to those who have suffered a fall without injury.

**Response24 is a FREE of charge service when signing up to the North Somerset Council Carelink service**

Support Broker Service – Personal Budgets & Self-funding Individuals

Our accredited support brokers can assist an individual who is self directing their support and/or in receipt of a Personal Budget to make the most of the resources available to them.

Our Support Broker Service can offer:  

  • Person cantered planning
  • Recruitment and management of Personal Assistants
  • Source care/support from agencies
  • Training for individual employers and/or their PAs
  • Applications for funding for the workforce
  • Payroll & managed account services
  • Find work and involvement in their local community
