Our Services include:

 

Community Care: We all want to live independently at home for as long as possible. At times, we may need assistance with daily living tasks. We believe you are the best person to determine the support you require to maintain independence in your home, that's why everything we do is centred around your personal needs and goals.

 

Overnight Care: Our specialised overnight team offers additional assistance during nighttime, ensuring uninterrupted, safe, comfortable sleep, whilst promoting independence and peace of mind.

 

Reablement Service: Our reablement service specialises in the rehabilitation of individuals who have recently been discharged from a hospital or healthcare setting, assisting them in retaining or regaining their skills and confidence.


Our ultimate objective is to reduce the readmissions to hospital by enabling our clients to regain their independence granting them the freedom to live life on their own terms.


At the end of a period of reablement, most people can live independently at home. However, if you are not fully independent or it is clear during your reablement that you need longer-term care and support, our professionals with guide you into your next steps.

 

End of Life Care: We provide a specialist care and support service to people who have a terminal illness or a progressive, incurable long-term condition. Our end of life service aims to help people die with dignity and our team work closely with Continuing Health Care professionals and multidisciplinary teams to arrange personalised packages of care.

 

Domestic & Support Services:

 

Cleaning: Whether you require a regular or even a one-time visit, we offer complete flexibility to suit your needs and budget (Please note minimum visit length is 1 hour).

 

Shopping: Keep your shelves stocked effortlessly with our unique shopping services. Whether you prefer traditional or online shopping, we have the perfect service to meet your needs. We complete the shopping, can put away shopping as required and can assist you to the shops, including transport wherever suitable or safe to do so.

 

Support Services: Our support services offer an extra helping hand beyond our regular care and domestic support services. These services are fully customisable to meet your specific needs and can be accessed as a one-time or as an ongoing service. This service can include transport wherever suitable or safe to do so.

 

Proactive & Responsive Services:

 

Response24 Service: Response24 is a 24-hour professional emergency first responder service which connects with any personal monitored alarm system, Telecare or Technology Enabled Care (TEC) equipment in North Somerset. It provides first responder assistance and offers options for year-round or weekly coverage based on your needs or those of a loved one.


During response calls we ensure peace of mind for individuals and family members with welfare checks, emergency personal care, and help for non-injury falls. We also work with many local professional and voluntary organisations and with consent we can signpost individuals, families, or carers for additional support to prevent deterioration or crisis. Please note this is a FREE service when purchasing the North Somerset Council CareLink personal alarm system.

Wellness Service

The Wellness service operates in the North Somerset area offering wellbeing telephone calls to people that require them.

The Wellness service aims to help improve and sustain the physical and mental wellbeing of people across North Somerset, allowing them to remain as independent as possible in their own home and communities.


The service is offered by Access Your Care in partnership with North Somerset Council and is entirely FREE to residents aged 18 and above who live in North Somerset .
